Employee injuries occurring during servicing or maintenance of machines or equipment from the unexpected energization or startup of the machines or equipment.
Requirements under OSHA's Control of Hazardous Energy (Lockout/Tagout) Standard, 29 CFR 1910.147
OSHA's Lockout/Tagout Standard requires employers to establish and implement an energy control program to ensure that a machine or equipment is isolated from the energy source and rendered inoperative before any employee performs any servicing or maintenance on the machine or equipment. Among other requirements, the employer must establish and implement procedures for safe:
- Shutdown and isolation;
- Application of lockout or tagout, as required by the standard;
- Relieving, disconnecting, restraining, and otherwise rendering stored or residual energy safe;
- Verification of isolation and deenergiztion; and
- Removal of lockout or tagout and restoration of energy. [29 CFR 1910.147]
The program must prevent the unexpected energizing or startup of the machines or equipment, or release of stored energy, that could occur and cause injury.
