Investigation Summary
At 4:55 p.m. on June 21, 2017, Employee #1, a power communications line technician, was servicing the electrical lines of a conveyor belt. As he was removing the insulation from a 270 Volt wire, his hand contacted the exposed wire and he was electrocuted.
