Investigation Summary
At 11:50 a.m. on July 11, 2017, a maintenance employee was cleaning the hopper of the PVC Recycle System on Line 1. The hopper is cleaned by opening the top and bottom butterfly valves of the hopper by pressing button on the solenoid and turning the air off with a side-to-side valve. Then the valves would be checked by hand to make sure those valves are opened and there are no obstructions. The line was not locked-out prior to cleaning the hopper or checking the valves. The employee reached under the lower valve through an opening in the pipe and touched the valve. The valve closed on the employee's hand. The employee sustained partial amputation of the middle and index fingers on the right hand. Note: The injured employee is right-handed.
