Investigation Summary
At 11:30 a.m. on September 21, 2017, Employee #1, an assistant mechanic, and a coworker were assigned to empty three side dump trailers (model number SDR 240-73) that were full of mud and water. Employee #1 and the coworker connected the necessary lines between the tractor and trailer, including an electric line for trailer lights, brake lines, and a hydraulic line to lift the trailers. The trailers use a large cylinder to push their center upward, causing a sideways tilt. While working to unload the first trailer, a malfunction occurred. Employee #1 exited the cab of the truck to investigate why the trailer was only rising at the front passenger corner. As Employee #1 stood between the trailer and its frame, manually overriding the system, the hydraulic pressure of the trailer was released and Employee #1 was crushed between the trailer and its frame. He was killed as a result.
