Investigation Summary
At 9:30 a.m. on September 24, 2017, Employee #1 and a coworker were making repairs to a burr house at a distillery. The coworker was using the mobile crane to lift pieces of sheet metal up to Employee #1, who was standing on top of an auger housing, on top of the burr house, attaching a C-clamp, with a rope tied to the C-clamp, onto the metal sheets to guide them into place. The employee had attached the C-clamp to a five foot by 10 foot metal sheet and was struggling to guide it through some framework of the structure. As Employee #1 was holding the rope that was attached to the C-clamp, he jerked the rope to get the metal sheet through the framework and the C-clamp came off. The employee fell backwards to the ground on the north side of the burr house and was killed.
