Investigation Summary
At 11:00 a.m. on October 6, 2017, an employee was climbing a ladder to place material in an overhead storage space at a indoor waste water treatment plant. Working alone, the employee was storing away left over ceiling tiles in an overhead storage space 11.5 feet above a level concrete floor. The employee had retrieved an 8 foot Werner Step Ladder from a storage shed for the non-routine task. During the climb, the ladder became unstable and tipped to the side. The employee fell and struck his head on the concrete floor and was killed. Factors that may have caused or contributed to the accident was: The storage area was at a height that required a longer ladder, the employee was carrying heavy and awkward material while climbing the ladder, and the employee would have a difficult time maintaining a 3 point contact when climbing the ladder as he handled the materials. In addition, would have had to over-reach outside the ladder side rails and lean to one side as he placed the material overhead.
