Investigation Summary
At 10:55 a.m. on November 15, 2017, an employee was placing an aluminum blank into the 200 ton Bliss mechanical straight side power press (model # SC2-200-60-48, serial # C-178-02; equipment #368). She pressed the lubrication button to lubricate the blank and then pressed the foot pedal. The press cycled and formed a baking sheet pan. The employee was standing in the gap between the frame of the press and the light curtains with her foot still on the foot pedal. The employee reached into the press to grab the baking sheet pan to slide it down the conveyor on the left side and the press cycled. Her hands were caught in the press and were amputated.
