Employee falls, hits head, and is killed

At 2:00 p.m. on March 7, 2018, per the employee's wife, the employee was retrieving tools/equipment from a work van when he lost his footing and fell backwards. He hit his head; however, did not report the incident. When he was hospitalized an approximate month later for a hematoma to his head, the employee died.
