Investigation Summary
At 4:00 p.m. on July 27, 2018, Employee #1 was operating a forklift, taking a load to the trash container. A cardboard box flew off while he was en route. The employee dismounted the forklift to pick up the box. A coworker, that was putting away a company truck, backed up and struck the employee while he was picking up the box that had fallen off of the forklift. Employee #1 was crushed between the company truck and forklift, and was killed.
