Investigation Summary
At 11:45 a.m. on July 30, 2018, Employer #1 lifted a 2012 Chevrolet Silverado truck, VIN number 1GCRCREA3CZ107385, five and a half feet off of the ground using a Hyundai wheel loader, Model HL 740TM-3, and Serial number L702GH10212. The employee crouched under the truck to remove the transmission cover to replace a transmission plug. The forks on the loader dropped, causing the truck to fall and crush Employee #1, killing him.
