Investigation Summary
At 1:10 p.m. on September 12, 2018, an employee was offloading miscellaneous hardware supplies from a 40-foot shipping container. Stored at the front of the container were seven solid core doors (positioned vertically) and approximately fifty boxes of various sizes stored next to the doors. During offloading of the boxes, the seven solid core doors fell and crushed the employee's head and neck. Total weight of the seven solid core doors was 672 pounds. The employee was killed.
