Investigation Summary
At 1:07 p.m. on January 7, 2019, an employee was in the process of repairing a rear pinion seal leak on a 2015 Ford Super Duty F-550 Truck. The employee removed the draft shaft and the truck rolled and pinned the employee between the truck and the rotary lift. The employee was killed.
