Investigation Summary
At 3:15 p.m. on January 13, 2019, Employee #1 parked the work van in the left lane of the two lane road. Employee #1 got out of the vehicle and proceeded to the back of the van to begin setting up traffic control in the 35 mph roadway. Employee #2 was sitting in the passenger seat of the van. As Employee #1 was placing cones at the rear of the work van, a 4-door sedan struck the employee, crushing the employee between the sedan and the work van. Employee #2 was in the passenger seat of the work van during the collision. Employee #1 was wearing personal protective equipment to include hardhat and reflective vest. Employee #1 sustained fatal "multiple blunt force injuries due to car-pedestrian collision" as per the Medical Examiner's Office's Autopsy Report.
