Investigation Summary
At 12:15 a.m. an employee was lifting drywall to approximately 7 feet high off the floor and began driving forward when a part of the bottom piece of drywall broke off and it fell to the floor. The employee stopped the reach truck, dismounted, and walked in front of the elevated load. The employee then attempted to pull off the remaining broken piece that was hanging from the bottom of the stack and was knocked to the ground when the stack fell from the forks, crushing and killing him.
