Investigation Summary
At 7:30 p.m. on September 16, 2019, an employee was working on top of a silo and had been taping a transfer pipe. At some point, the employee fell through the guardrail openings and struck the ground, killing him. Injuries included a fractured pelvis, dislocated bones, and head and neck lacerations.
