Investigation Summary
At 4:18 p.m. on September 24, 2019, an employee was working from a ten foot A-frame ladder to install three foot Aluminum Jacketing insulation on a chilled water pipe. The employee fell off of the ladder and received several rib fractures along with multiple internal injuries from blunt force trauma that killed him.
