Investigation Summary
At 9:00 a.m. on October 4, 2019, Employee #1 was operating a forklift. The employee observed a pallet jack inside of a trailer of a truck that was moving while the truck was parking. He jumped out of the forklift to stop the pallet from falling out of the trailer. The forklift was not fully stopped and rolled forward. The employee became pinned between the truck and the forklift, and was killed.
