Investigation Summary
At 3:30 p.m. on February 13, 2020, an employee was climbing on the end bracing of a scaffold while waiting for a concrete truck. At some point, either ascending or descending the scaffold (no witnesses were present), the employee fell approximately eighteen feet and was killed due to blunt force trauma to the head.
