Investigation Summary
An employee was working at a specialty hospital. It provided sub-acute care, long term care, hospice and palliative care, and respite care. The employee worked as a licensed practical nurse (LPN). He contracted COVID-19. At 8:00 a.m. on April 11, 2020, he died. On or about June 3, 2020, the employer became aware that the employee had died. The employer did not report the fatality to OSHA until June 8, 2020. This was an OSHA-covered fatality.
