Investigation Summary
At approximately 2:30 p.m. on June 16, 2020, an employee was performing assigned duties as a restorative certified nursing assistant for the employer. The employee worked during the two weeks prior to the onset of the symptoms on June 6, 2020. The employee contracted COVID-19, was hospitalized on June 7, 2020, and then died on June 16, 2020.
