Investigation Summary
An employee was working at a nursing care or skilled nursing facility. She provided nursing care to patients that had possibly been infected with the COVID-19 virus. One of the residents that was sent to the hospital on June 12, 2020, was hospitalized and tested positive. The facility was not notified at that time. On June 18, 2020, the employee tested positive for COVID-19. At 2:15 p.m. on June 28, 2020, she died.
