Investigation Summary
At approximately 12:00 a.m. on August 5, 2020, an employee was the Director of Staff Development/Training and was exposed to COVID-19 possibly at work. Twenty seven other employees tested positive for COVID-19 at the facility. The employee was diagnosed on June 20, 2020 and admitted to the hospital the same day. The employee's last day was June 12, 2020. She still showed symptoms on June 15, 2020 with a confirmed COVID-19 positive results on June 19, 2020. She was hospitalized on June 30, 2020 and died on August 5, 2020.
