Investigation Summary
Employee # 1 was a warehouse agent in the client company, where he had been assigned by his temporary agency employer. He worked during the two weeks prior to the onset of his symptoms on August 22, 2020. Employee # 1 contracted COVID-19. He was never hospitalized. At 2:45 p.m. on August 31, 2020, he died at his house. His employer reported the fatality to the state OSHA on September 2, 2020. An illness investigation report from the client company shows that there were more COVID-19 cases at the site around the time employee contracted COVID-19, and Employee # 1 was identified as a contact for one of the positive cases. The report also mentioned that Employee # 1 may had been extra-occupationally exposed while carpooling with co-workers who tested positive for COVID-19.
