Investigation Summary
At approximately 12:00 p.m. on September 16, 2020, an employee worked as a Certified Nursing Assistant working at the skilled nursing facility. They have a COVID 19 positive ward, but employee did not work there. It was the employee's sixth day back from working from a two-week vacation. The employee was feeling "cold" told the supervisor on August 24, 2020. The employee tested positive for COVID-19 on August 26, 2020, and was hospitalized subsequently. The employee died from COVID-19 on September 16, 2020.
