Investigation Summary
Employees #1 and #2 were employed by a pharmaceutical manufacturer. Employees worked on production lines and in offices less than six feet apart without any engineering controls in place. Employees entered and exited the facility without following proper screening procedures. Employees wore face masks incorrectly and in some cases not at all in the shipping warehouse and throughout facility. Over 37 employees tested positive for COVID-19 while working on the first shift. Employee #1 was employed as a facility manager and began to experience symptoms of COVID-19 on October 19, 2020. The employee's last day of work was on October 21, and his COVID-19 test returned positive on October 23. The employee was hospitalized on November 1, and he died on November 16, 2020. Employee #2 worked in the shipping warehouse and began to experience symptoms of COVID-19 on November 26. Employee #2's last day of work was on December 3, and he was tested for COVID-19 on December 4. Employee #2's results came back positive on December 5, when he became hospitalized. Employee #2 successfully recovered from his infection.
