Investigation Summary
At approximately 7:30 a.m. on December 2, 2020, an employee worked in a residential outpatient recovery services facility. The Division staff contacted the employer on December 2, 2020to begin the opening conference process. The employee took a rapid test on November 16, 2020, and was required by the employer to quarantine at home where the employee's sister monitored his condition after testing positive. The employer was notified that the employee died on December 2, 2020, at home.
