Investigation Summary
An employee worked as a Certified Nursing Assistant caring for residents in a skilled nursing facility. On December 12, 2020, the employee had taken a routine bi-weekly COVID-19 test and tested positive. The employee began in-home quarantine the same day. On December 21, 2020, while nearing the end of her quarantine the employee began experiencing symptoms consistent with COVID-19. The employee continued to quarantine and at 8:00 p.m. on December 27, 2020, she was discovered on the bathroom floor of her home. While enroute to the hospital via ambulance, the employee experienced a stroke and died the same day in the hospital. The date, time and other details leading up to exposure are not indicated in the narrative. The employer did not have an adequate Respiratory Protection Program in place.
