Investigation Summary
At 8:00 a.m. on December 28, 2020, an employee was operating a Rotary Slotter Machine that was low on ink. While the machine was still running, the employee began adding thick and sticky ink using a dispensing paddle. In the process, the employee's hand, wrist, and forearm became caught between the transfer roller and the print roller. The employee sustained a fractured finger and severe lacerations. The employee was hospitalized.
