Investigation Summary
An employee arrived at work at a savings bank on December 10, 2020, with a sore throat and she was sent home by her manager and instructed to get tested for COVID-19. On December 11, 2020, the employee got tested for COVID-19 and received a positive result the next day. On December 19, 2020, the employee was hospitalized as a result of COVID-19, and on December 30, 2020, the employee died. An assistant manager at the location was exposed to a family member on December 8, 2020, self-quarantined and was tested for COVID-19 on December 9, 2020 and received a positive test result. The location manager, upon hearing about the positive result from the assistant manager, shut down the location on December 11, 2020 and it remained closed for a deep cleaning until December 14, 2020. Additionally, the three remaining employees at the location were tested for COVID-19 on December 14, 2020, with two employees testing negative and one employee testing positive.
