Investigation Summary
In December 2020, an employee was working as a licensed practical nurse at a nursing care or skilled nursing facility. Before work on December 7, 2020, she performed a nasal swab for COVID-19. The test results were negative. On the following day, December 8, she showed up at work symptomatic for COVID-19. A nasal swab rapid test on this day showed positive results for COVID-19. The employee was sent home to quarantine. She never returned to the facility. She was reportedly hospitalized off and on for almost two months before dying in the hospital, on February 6, 2021. The staff at the facility primarily wore surgical masks until the first resident tested positive for COVID-19, on November 29, 2020. From that point on until February 2021, the facility required all employees to wear N95 respirators. The employer reported that 21 of the facility's 38 employees eventually tested positive for COVID-19. This was an infection rate of 55 percent. All residents in the facility except one contracted COVID-19.
