Investigation Summary
An employee was working for an insurance agency that provided various services to its members, such as insurance, travel, and roadside assistance services. He worked in a large, three-story building as a roadside service dispatcher. He was not vaccinated. On August 27, 2021, while at work, he was observed coughing. He was instructed to go home. On or about August 29, 2021, he was said to have sought treatment for symptoms associated with COVID-19 at a U.S. Department of Veterans Affairs (VA) hospital. It was there that he was tested for COVID-19. He learned that same day that his test results were positive. According to his next of kin, the employee was sent home with medication to treat a pneumonia-like illness and to convalesce. While at home on September 9, he fell or passed out in the bathroom. Emergency medical services were called. The responders were able to revive the employee temporarily, but while in transport or shortly after arriving at the hospital, he died. His cause of death was reported as acute respiratory failure with hypoxia due to COVID-19. At 8:00 a.m., the employer was notified on the following day, September 10, that the employee had died due to complications of COVID-19. About three other employees have tested positive at the worksite. The employer had not been able to speak with the employee before he died to gather information on where he suspected he contracted COVID-19.
