Investigation Summary
On December 11, 2021, an employee who worked in patient healthcare at a residential intellectual and developmental heathcare facility died from COVID-19. On November 3, 2021 and November 4, 2021, the employee worked with a coworker who later tested positive for COVID-19 on November 5, 2021. While interacting at work, the employee and the coworker failed to wear a NIOSH-approved respirator. The employee was not placed on quarantine after being in close contact with the COVID-19 infected coworker. The employee initially tested positive for COVID-19 on November 14, 2021, and was off work until November 24, 2021. On December 5, 2021, the employee called in sick and reported going to the hospital. On December 6, 2021, the employee was hospitalized. The employer did not have an adequate Respiratory Protection Program in place.
