Investigation Summary
An employee was working as a social worker whose job required her to go out to youth placement facilities to check on clients once per month and also visit group homes. The employee had a case load of 30 clients and visits to the various sites ranged from 30 minutes to 2 hours. The employee had worked in the office from November 1-19, 2021, with one telework day during that period. The employee visited a family home on November 3, 2021 but the family did not report any cases of COVID-19 during that time. Another visit was made on November 18, 2021, at a foster family home, but they did not have any reports of COVID-19 infections. There was one case of COVID-19 in a coworker around the time that the employee became ill. The coworker tested positive on November 23, 2021. The coworker sat next to the employee in the office and reported that they wore their masks and maintained a social distance of 6 feet. The employer had their employees go through online training and also had meetings to discuss the written COVID-19 prevention program. KN95 masks were required along with the use of gloves and social distancing if having to go indoors. If suspected contact or known to have tested positive for COVID-19, a face shield, gown and gloves were required to be worn and social distancing practiced. Cloth masks were required if not entering indoors. Interviews were to be conducted outdoors while observing social distancing. Employees were provided with disinfectant spray and paper towels to clean their vehicles. The employee was diagnosed with hypoxemic respiratory failure secondary to COVID-19 pneumonia and died on December 29, 2021.
