Investigation Summary
Employee #1 (owner/operator) was instructing his grandson how to off-load a John Deere skid steer off of an equipment trailer, while he standing on the fender of the trailer. The grandson lost control of skid steer while operating it and knocked Employee #1 off of the equipment trailer, and the skid steer tipped over onto Employee #1, crushing him. Employee #1 was taken to MCV Hospital, where he died 16 days later.
