Investigation Summary
At 7:00 a.m. on September 30, 2022, an employee was performing maintenance on the North Side Ultrafast Elevator (Autoquip-American Lifts: Tork lift) in the picking aisle of a refridgerated meat storage warehouse. The employee entered the cage and depressurized the hydraulic line causing the platform to collapse on top of him. The employee's abdoman was crushed by the equipment and killed.
