29 CFR 1904.39(a): The employer did not report the death of an employee or the in-patient hospitalization of one or more employees because of a work-related incident: a) 750 W. Berkley Ave., Norfolk, VA 23523: On or about January 20, 2015, and times prior thereto, the employer failed to report to OSHA within 24 hours an in-patient hospitalization of an employee who sustained work-related injuries while performing work using a lathe. No abatement documentation or certification required.
