Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause serious physical harm to employees in that employees were exposed to: (a) On or about June 19, 1995, the manually operated slide at the bottom of the Lowry bin was not in a closed and locked position to prevent movement of beans while employees were working inside the bin, exposing them to potential engulfment hazards. Among other methods, one feasible and acceptable abatement method to correct this hazard would be close and lockout the slide when employees are in the bin or not allow employees in the bin while beans are being drawn out the bottom during bag filling operations.
