Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. The employer fails to assure effective protection against amputation injury at rotating parts as required by the standard, as demonstrated on January 31, 2017 when an employee working at a Carlton Helical Technologies, Depucking Machine, Machine Number 1, at 1190 North US Highway 1, Ormond Beach, FL 32174 placed her hand against the side pulley and belt guard of the machine, and subsequently wrapped her fingers around the guard causing her left index and left ring finger to be crushed and amputated by the conveyor belt, pulley belt and sheave of the machine. Edgewell Personal Care, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 29, 1910.212(a)(1), which was contained in OSHA inspection number 1126114, citation number 1, item number 2 and was affirmed as a final order on April, 13, 2016, with respect to a workplace located at 10 Leighton Road, Milford, CT 06460.
