29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not conducted at least once every 3 years: The employer is failing to protect employees from Struck-by hazards while operating, and being in close proximity to, a Komatsu forklift (SN-515616A Model FG25HT-12). This was most recently documented on or about 3/16/2017 at the location of 2300 E. Trail in Dodge City KS. Employees were exposed in that an evaluation of the operator's performance was not conducted at least once every 3 years.
