29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about March 15, 2017, in the fittings department located in building (6) six, the employer did not ensure the Lown Slip Roll Forming & Plate Bending Machine, Model B300, S/N B-236563 points of operation were guarded exposing employees to caught-in hazards. The M&M Manufacturing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1095292, citation number 01, item number 001 and was affirmed as a final order on 01/21/2016 with a final abatement date of 01/20/2016, with respect to a workplace located at 4001 Mark Iv Parkway, Fort Worth, TX 76106.
