Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burns and smoke inhalation from fires and explosion due to the use of unapproved electrical equipment: a)Brooder Barn #1, First Floor - The raceway covers were not on both ends of the electrical raceway at the first fan bank. b)Brooder Barn #1, Second Floor - The raceway covers were not on both ends of the electrical raceway at the first fan bank. c)Brooder Barn #1, Third Floor - The raceway covers were not on both ends of the electrical raceway at the first fan bank. d)Complex 40, Barn 47 - The cover was off the electrical raceway on both ends and the motor starter box cover was not closed. e)Complex 40, Barn 47 - The covers were off 3 motor starter boxes at the fourth fan bank. f)Barn 86, South Walkway - The electrical box mounted on the wall and containing a light fixture was not fully enclosed to prevent dust contamination. g)Barn 84, South Walkway - The wall mounted light fixture was defective and not contained within an approved enclosure. h)Barn 84, North Walkway - The wall mounted light fixture was not securely mounted to the electrical box to prevent dust contamination. i)Barn 80, East Walkway - The ceiling mounted light fixture was not secured to the electrical box to prevent dust contamination. j)Barn 86, Middle Ceiling - The 12 receptacle boxes supplying power to the mechanical feeders were not contained within approved enclosures and covers. k)Barn 85, West End -
