Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burns and smoke inhalation from fires and explosion due to the use of unapproved electrical equipment: a)Brooder Barn #1, First Floor - The raceway covers were not on both ends of the electrical raceway at the first fan bank. b)Brooder Barn #1, Second Floor - The raceway covers were not on both ends of the electrical raceway at the first fan bank. c)Brooder Barn #1, Third Floor - The raceway covers were not on both ends of the electrical raceway at the first fan bank. d)Complex 40, Barn 47 - The cover was off the electrical raceway on both ends and the motor starter box cover was not closed. e)Complex 40, Barn 47 - The covers were off 3 motor starter boxes at the fourth fan bank. f)Barn 86, South Walkway - The electrical box mounted on the wall and containing a light fixture was not fully enclosed to prevent dust contamination. g)Barn 84, South Walkway - The wall mounted light fixture was defective and not contained within an approved enclosure. h)Barn 84, North Walkway - The wall mounted light fixture was not securely mounted to the electrical box to prevent dust contamination. i)Barn 80, East Walkway - The ceiling mounted light fixture was not secured to the electrical box to prevent dust contamination. j)Barn 86, Middle Ceiling - The 12 receptacle boxes supplying power to the mechanical feeders were not contained within approved enclosures and covers. k)Barn 85, West End - The electrical control and junction boxes mounted on the waste scraping and egg conveying framework were missing 32 knockouts. l)Barn 85, Middle Ceiling - The 12 receptacle boxes supplying power to the mechanical feeders were not contained within approved enclosures and covers. m)Barn 84, West End - The electrical control and junction boxes mounted on the waste scraping and egg conveying framework were missing 37 knockouts. n)Barn 84, West End Ceiling - The 2 receptacle boxes supplying power to the feed augers were not contained within approved enclosures and covers. o)Barn 84, Middle Ceiling - The 12 receptacle boxes supplying power to the mechanical feeders were not contained within approved enclosures and covers.p)Montville, Brooder Barn - In the boiler room between barns 967-968 the #5 circulator had no face cover plate to contain its electrical wires. q)Barn 83, Middle Ceiling - The 12 receptacle boxes supplying power to the mechanical feeders were not contained within approved enclosures and covers. r)Barn 82, Middle Ceiling - The 12 receptacle boxes supplying power to the mechanical feeders were not contained within approved enclosures and covers. s)Barn 84, Southwest Fan Room - The electrical box containing the 240 volt fan control units was not provided with a cover. t)Montville, Brooder Barn - The diesel generator in the generator room between barns 967-968 had no electrical panel shield for the electrical junction box on the right. u)Barn 81, Middle Ceiling - The 12 receptacle boxes supplying power to the mechanical feeders were not contained within approved enclosures and covers. v)Complex 10, Barn 6 - The junction box was knocked off the motor for pit scraper unit No.1. w)Egg Processing Plant #7 - The covers were open on the control panels for egg packing machines #1, 2, 4, 5, 6, 7, 8, and 9. x)Complex 70, Barn 76 - The junction boxes were knocked off both of the motors for the two pit scrapers. y)Complex 70, Barn 70 - The junction box was knocked off the pit scraper motor of the #2 unit. z)Complex 70, Barn 72 - The junction boxes were knocked off both of the motors for the pit scrapers. a2)Complex 10, Barn 1 - The junction box on pit scraper unit #2 motor was damaged. b2)Complex 10, Barn 2 - The junction box on pit scraper motor No.2 was severely damaged. c2)Complex 10, Barn 3 - The 3 junction boxes for the 3 pit scraper motors were knocked off the motors. d2)Complex 10, Barn 4 - The junction box for pit scraper motor #1 was knocked off the motor. e2)Complex 10, Barn 7 - The 3 junction boxes for the 3 pit scraper motors were knocked off the motors. f2)Complex 10, Barn 9 - The junction boxes on pit scraper motors #1 and 2 were damaged with the conductors hanging out of the boxes. g2)Complex 10, Barn 10 - The junction box on pit scraper motor #1 was damaged. h2)Complex 10, Barn 12 - The junction boxes on pit scraper motors #1 and 2 were damaged. i2)Complex 70, Barn 72 - Two fan switches at the 3rd fan bank had broken covers. j2)Complex 70, Barn 72 - One fan switch cover at the 4th fan bank was broken. k2)Complex 50, Barn 48 - The 8 electrical receptacle boxes for the automatic cage feeders had their electricalwires hanging outside the receptacle boxes and had no receptacle face plate covers. l2)Complex 50, Barn 49 - The 8 electrical receptacle boxes for the automatic cage feeders had their electrical wires hanging outside the receptacle boxes and had no receptacle face plate covers. m2)Complex 50, Barn 50 - The 8 electrical receptacle boxes for the automatic cage feeders had their electrical wires hanging outside the receptacle boxes and had no receptacle face plate covers. n2)Complex 50, Barn 51 - The 8 electrical receptacle boxes for the automatic cage feeders had their electrical wires hanging outside the receptacle boxes and had no receptacle face plate covers. o2)Complex 50, Barn 53 - The 8 electrical receptacle boxes for the automatic cage feeders had their electrical wires hanging outside the receptacle boxes and had no receptacle face plate covers. p2)Complex 50, Barn 54 - The electrical shocker feeder junction box on the south end of the second floor had no face cover plate to contain its wires. k3)Snells Hill Road, Brooder Barn 3 - The electrical shocker feeder junction boxes of rows 1-2-3 of the second floor had no face cover plates to contain their electrical wires. l3)Snells Hill Road, Brooder Barn 3 - The electrical shocker feeder junction box in row 1 on the first floorhad no face cover plate to contain its electrical wires. m3)Snells Hill Road, Brooder Barn 4 - The electrical junction box for the automatic cage feeders on the north end of the third floor had no face plate covers to contain their electrical wires. n3)Snells Hill Road, Brooder Barn 4 - The electrical control junction box for the automatic cage feeder motor on the north end of third floor did not have a face cover plate to contain its wires. o3)Snells Hill Road, Brooder Barn 4 - The electrical control switch for the automatic cage feeder on the west side of row 3 on the third floor had no face cover plate to contain its wires. p3)Snells Hill Road, Brooder Barn 4 - The electrical junction boxes for the automatic cage feeders of rows1-2-3-on the first floor had no face cover plates to contain their electrical wires. q3)Snells Hill Road, Brooder Barn 3 - The electrical junction boxes for the automatic cage feeders of rows1-2-3-on the third floor had no face cover plates to contain their electrical wires. r3)Snells Hill Road, Brooder Barn 3 - On the north end of row 1, on the third floor the electrical control switch box for the automatic cage feeder had no face cover plate to contain its electrical wires. s3)Poland, Brooder Barn 3 - The electrical junction box for the heater in the packing area had no face plate cover to contain its electrical wires. t3)Pittsfield, Brooder Barn - On the east wall of the third floor the 3 electrical switch control boxes for the exhaust fan bank had no face cover plates to contain their electrical wires. u3)Pittsfield, Brooder Barn - The six electrical junction boxes for the automatic cage feeders on the third floor had no face cover plates to contain their electrical wires. v3)Pittsfield, Brooder Barn - At the north end of fourth floor two electrical control switch boxes for the automatic cage feeders had no face cover plates to contain their electrical wires. w3)Pittsfield, Brooder Barn - At the north end of the fourth floor the electrical control switch box for the exhaust louver had no face cover plate to contain its electrical wires. x3)Pittsfield, Brooder Barn - At the northeast end of the second floor two electrical junction for the automatic cage feeders had no face cover plates to contain their electrical wires. y3)Pittsfield, Brooder Barn - On the northwest side of the second floor an electrical junction box for the automatic cage feeder had no face cover plate to contain its electrical wires.z3)Pittsfield, Brooder Barn - At the north end of the first floor six electrical junction boxes for the automatic cage feeders had no face cover plates to contain their electrical wires. a4)Meadowbrook, Brooder Barn - At the north end of the third floor two electrical junction boxes for the automatic cage feeders had no face cover plates to contain their electrical wires. b4)Meadowbrook, Brooder Barn - At the north end of the second floor two electrical junction boxes for the automatic cage feeders had no face cover plates to contain their electrical wires. Among other methods, one feasible and useful method of abatement would be to install gaskets and covers to render electrical equipment dust tight.
