Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards. (a) On or about July 6, 2017, employees were in the process of an EMI scan of a drill head stem pipe. The pipe was supported by two hydraulic pipe jacks. An employee was positioned between the pipe being inspected and another pipe set on layout inspection rack. One of the pipe jacks tipped, causing the pipe being inspected to fall, resulting in the employee being caught between that pipe and the pipe positioned on the layout rack.
