29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: 1) Third Floor, Westside of the building: On or about July 5, 2017, and at times prior, the employer exposed employees to struck-by hazards while lifting metal panels weighing approximately 850 to 1,100 pounds, in that personal protective equipment (PPE) such as hard hats were not worn.
