29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Facility, 4170 Pine Hollow Road, Trafford, PA: On or about August 10, 2017, the employer did not utilize a hazardous energy control procedure to prevent the unexpected release of stored potential (gravitational) energy while removing the counterweight of a Caterpillar 374FL excavator. An employee was struck and killed by the falling counterweight.
