29 CFR 1910.178(l)(4)(i): Refresher training, including an evaluation of the effectiveness of that training, shall be conducted as required by paragraph (l)(4)(ii) to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely. a) Inside the warehouse- The employer did not provide refresher training to employees who were operating powered industrial trucks; on or about 10/05/17. b) Inside the warehouse- The employer did not provide refresher training to employees who were operating powered industrial trucks; on or about 10/11/17. Repeat: St. George Logistics was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(4)(i), which was contained in OSHA inspection number 989952, citation number 1, item number 1, and was affirmed as a final order on 2/10/15, with respect to a workplace located at 5 Logistics Drive, Kearny, NJ 07032.
