29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about January 8, 2018 at the jobsite on the south side of the new home; where employees were accessing the second story roof with an extension ladder that did not extend at least 3 feet above the landing surface exposing employees to falling approximately 20 feet to the lower level. (b) On or about January 8, 2018 at the jobsite on the east side of the new home; where employees were accessing the second story roof with an extension ladder that did not extend at least 3 feet above the landing surface exposing employees to falling approximately 20 feet to the lower level. (c) On or about January 8, 2018 at the jobsite on the north side of the new home; where employees were accessing the first story roof with an extension ladder that did not extend at least 3 feet above the landing surface exposing employees to falling approximately 10 feet to the lower level. (d) On or about January 8, 2018 at the jobsite on the west side of the new home; where employees were accessing the second story roof with an extension ladder that did not extend at least 3 feet above the landing surface exposing employees to falling approximately 20 feet to the lower level.
