29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) 3399 Forsythia Dr., Odessa Fl - employees engaged in roofing activities on a 5 in 12 sloped roof were exposed to a 10'5" fall hazard, in that, fall protection equipment was not installed or utilized, observed on or about 2/1/2018. Ad-Ler Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1139096, citation number 1, item number 1 and was affirmed as a final order on 05/12/2016, with respect to a workplace located at 8219 Enclave Way, Sarasota, FL, 34243. Ad-Ler Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1069360, citation number 1, item number 1 and was affirmed as a final order on 08/05/2015, with respect to a workplace located at 3635-3665 Bonita Beach Blvd, Bonita Springs, FL, 34134.
