Text For Citation: 01 Item/Group: 001 Hazard:
OSHA Act of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the recognized hazards of burn and/or struck by injuries in the event of catastrophic equipment failure and subsequent fires while testing and inspecting industrial furnaces: (a) Location 60 Fitchburg Road, Ayer, MA: On or about March 23, 2018, employees were performing tests and inspections to commission a hot isostatic pressing (HIP) furnace and were exposed to hazards from catastrophic failure of the HIP furnace. These hazards include but are not limited to fire and explosion.
