29 CFR 1910.333(a)(1): Live parts that the employee was exposed to were not de-energized before the work began in or near them. (a) Mechanical Room: On or about March 15, 2018, the employer exposed employees to arc flash hazards in that, work was performed to upgrade a live 480 volt electrical panel at the switchgear to build out an office space and it was not de-energized.
