29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used for access. A) Casey Bortles on or about April 26, 2018 at the site located at 1466 Blackhawk Drive, Waterville, OH: The employer did not ensure a means of access was provided for an employee working from a ladder jack scaffold 17ft above the level below. The exposed employees were climbing the supporting ladders to access and exit the ladder jack platform. More than two feet separated the access ladder rung and the working platform.
